Responsibilities

  • Effectively and efficiently provide treatment interventions for short-term and long-term rehab.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Administer a wide variety of interventions as part of the plan of care based upon individual patient needs.
  • Assess patient’s progress and collaborate with physical therapists on recommendations for adjustments.
  • Provide consultation and counseling to patients and their caregivers.
  • Effectively communicate with the OneClinical Team regarding patient progress.
  • Collaborate with physical therapists for patient performance updates and discharge success.

Duties

  • Utilizes standardized assessments and evidence-based practice to support clinical interventions
  • Administers a wide variety of interventions as part of the plan of care based upon individual patient needs
  • Assesses patient’s progress and collaborates with physical therapists on recommendations for adjustments to plan of care per regulatory and clinical practice requirements
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders
  • Effectively communicates with the OneClinical Team regarding patient progress and barriers
  • Collaborates with physical therapists for progress of patient performance, updates to plan of care, and discharge success
