Qualifications

• Certified Physical Therapist Assistant license required (US).
• Experience delivering patient-centered interventions for seniors in SNF or post-acute settings.
• Ability to document patient progress and outcomes in EMR.

Responsibilities

• Deliver evidence-based interventions under the supervision of a Physical Therapist.
• Assist patients with mobility and activities of daily living to improve function.
• Document caseload, progress, and plan of care in the EMR and collaborate with the interdisciplinary team.
