Job summary

Choice Health At Home seeks a qualified Physical Therapist Assistant (PTA) for a PRN role in The Woodlands, TX. The PTA supports licensed physical therapists in delivering rehabilitative care, implements treatment plans, and monitors patient progress to optimize recovery outcomes.

Responsibilities include conducting exercises, managing modalities, educating patients and families, and maintaining a safe clinical environment. Prior home health experience and AHA CPR certification are preferred.

Qualifications

• Successful completion of accredited physical therapy program and state licensure exam.
• Licensed PTA and CPR certification are required.
• Home health experience is preferred.
• Knowledge of home health care best practices.
• Ability to educate patients and families on exercises and safety.

Responsibilities

• Assist physical therapists in executing individualized treatment plans for patients with musculoskeletal and neurological conditions.
• Conduct therapeutic exercises, manual therapy techniques, and modalities under supervision.
• Monitor and document patient responses to treatment and report progress to the supervising therapist.
• Educate patients and families on exercises, proper device use, and injury prevention.
• Maintain a safe and clean treatment environment and ensure equipment is sanitized.

This position involves implementing treatment plans designed by physical therapists, monitoring patient progress, and adjusting therapeutic activities to optimize recovery outcomes. The PTA works closely with patients to improve mobility, reduce pain, and restore function through therapeutic exercises, manual therapy, and the use of specialized equipment. Effective communication with patients, families, and the healthcare team is essential to ensure coordinated care and patient education. Ultimately, the PTA contributes significantly to enhancing patients' quality of life by facilitating their return to daily activities and independence.

Responsibilities

• Maintain a safe and clean treatment environment, ensuring all equipment is properly sanitized and in good working order.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy Assstant license

1 year of home health experience preferred

AHA CPR certification

Valid Driver's License

Valid Auto Insurance

HomeCare HomeBase experience preferred
