FeldCare Connects is seeking a self-motivated Physical Therapist Assistant for the Los Angeles area on an Independent Contractor basis. You will assess and treat adults in home health, craft individualized plans, and monitor outcomes to promote mobility and independence.

Work flexible hours, be your own boss, and collaborate with a supportive team while maintaining high-quality documentation and communication with supervising therapists.

Formación

• Must be able to work with adults of all ages.
• CA PT license and active registration required.
• Curriculum approved by APTA or equivalent bodies; bilingual is a plus.
• Ability to document patient care with accuracy and discretion.
• Sound clinical reasoning in planning, treatment, and progress.
• Recognize when to modify protocols to meet patient needs.

Responsabilidades

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function.
• Develop and execute a treatment plan and interventions; monitor progress.
• Document care accurately and communicate findings to supervising therapist; adjust plans as needed.
• Maintain accurate, discreet patient records.
• Communicate effectively with patients and care team.
• Operate as an Independent Contractor delivering home health services in the LA area.

This position is for an Independent Contractor to serve the Los Angeles area specifically.

FeldCare Connects is currently seeking a self-motivated Physical Therapist Assistant to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

Physical Therapist Assistant in Home Health is responsible for the assessment and evaluation of patient care needsto promote the ability to move, reduce pain, restore function, and prevent disability. Based on this assessment and evaluation, the Physical Therapist determines a treatment plan,executes interventions aimed at improving and enhancingthe patient's well-being, and monitors the patient's progress effectively.

A Physical Therapist Assistant for Home Health must have:

• Remarkable knowledge of physical therapy principles, theories, methods in their application in a rehabilitation environment.
• Great skills in providing a variety of treatments.
• Profound ability to communicate effectively both written and oral.
• Exceptional ability to understand the role of therapy in the context of the patients age, needs and environment.
• Exceptional ability to prepare and maintain records with accuracy and discretion.
• Proven sound clinical reasoning in treating and planning, implementation and monitoring patient progress.
• Ability to recognize when patient protocols or treatment plans need modification to meet appropriate patient needs and report to the therapist.

Qualifications:

• Ability and enthusiasm in working with all ages
• Physical Therapistlicense and registration by the state
• Completion of a Physical Therapistcurriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association
• Bilingual is a plus!
