Job Overview

Evolve Therapy Services is a therapy management organization hiring compassionate therapists for our partner located in Olivia, MN.

Position: Physical Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time or PRN

Purpose: To assist in planning, organizing, developing, and directing Physical Therapy services, ensuring high-quality resident care in compliance with applicable standards and regulations.

Responsibilities
  1. Provide physical therapy services to residents under the supervision of the Physical Therapist.
  2. Conduct regular screenings to determine residents' needs for intervention or treatment.
  3. Assist in evaluations and develop treatment plans with the Physical Therapist.
  4. Conduct individual and group treatments, focusing on areas such as muscle strength, mobility, balance, gait, and modalities.
  5. Select and adapt therapy techniques/media based on resident performance.
  6. Monitor residents' responses and consult with the Physical Therapist for modifications.
  7. Assist in establishing and adjusting resident schedules and educate residents, families, and staff on equipment use.
  8. Participate in quality assurance and complete compliant documentation in NetHealth EMR systems.
Qualifications
  • Associates Degree in Physical Therapy from an accredited program.
  • Active Physical Therapy Assistant license in the practicing state(s).
