Job Purpose

Assist in planning, organizing, developing, and directing Physical Therapy services to ensure high-quality resident care, following all applicable standards and regulations.

Key Responsibilities

• Provide physical therapy services under the supervision of a Physical Therapist.
• Conduct resident screenings and assist in evaluations.
• Help develop and implement treatment plans with measurable goals.
• Perform individual and group treatments, focusing on areas such as mobility, strength, balance, and gait training.
• Monitor resident responses and adjust treatments accordingly.
• Train residents, families, and staff on equipment use.
• Participate in resident and staff education programs.
• Complete accurate documentation in compliance with CMS and other standards.

Qualifications

• Associates Degree in Physical Therapy from an accredited program.
• Active Wisconsin Physical Therapy Assistant license.
