Job summary

ABL Health Care invites applications for a Full-Time Physical Therapist Assistant (PTA) to join our Home Health team in San Leandro. The PTA will evaluate patients, develop and implement individualized treatment plans, and collaborate with the interdisciplinary team to improve mobility and quality of life.

Travel within a 15-25 mile radius is required, with remote support and field visits as part of the role.

Qualifications

• Current Physical Therapist Assistant (PTA) License to practice in California - Required
• Experience in Home Health care - 1 Year Required
• Current CA Driver’s License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Position Overview:

We are seeking an experienced, self-driven Full Time Physical Therapist Assistant (PTA) for Health Care to join our dedicated team and provide exceptional care throughout the region. As a PTA, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.
