You will collaborate closely with our Clinical Director and clinical staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan in coordination with the interdisciplinary team.
• Administer treatments, monitor patient conditions, and report signs or symptoms indicating a change in patient condition to physician, case manager, or as needed to the director of nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current Physical Therapist Assistant (PTA) license to practice in California – required.
• One year of experience in home health care – required.
• Current California driver’s license.
• Ability to travel within a 15–25 mile radius on a regular basis.
