Job Summary:

The PTA administers physical therapy to patients in their homes based on physician orders and under the supervision of a Physical Therapist. Responsibilities include implementing care plans, educating patients and families, and participating in care coordination and improvement activities.

Responsibilities:
  • Adhere to agency policies and procedures
  • Provide prescribed physical therapy as per plan of care
  • Assist patients in improving physical functions and minimizing disabilities
  • Participate in care planning and team meetings
  • Document patient progress and update care plans timely
Requirements:
  • Graduate of an accredited two-year college program approved by the American Physical Therapy Association
  • Current licensure in practicing state(s)
  • Preferred: Two years of experience
  • Ability to work independently and exercise judgment
Physical Requirements:
  • Ability to participate in physical activities including lifting, bending, and standing for extended periods
