Sun Recruiting, Inc. is seeking a dedicated Physical Therapist Assistant to provide in-home physical therapy services in San Jose, CA. You will deliver targeted interventions and develop personalized treatment plans to enhance patients’ mobility and quality of life.

The role requires CA PTA licensure, fingerprint clearance, CPR/ACLS, up-to-date TB test and flu shot, and comprehensive immunization records. A valid driver’s license with auto-insurance is also required.

Qualificações

• Licensed PTA in California as a prerequisite.
• CA Fingerprint Clearance Card required.
• CPR/ACLS certifications must be current.
• TB test result within the past year and flu vaccination.
• Immunization records including Covid19 and Hep B are required.
• Valid driver’s license and auto-insurance must be maintained.

Key Responsibilities

• Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
• Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
• Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.

Required Qualifications

• Current Physical Therapist Assistant license in the State of California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
