Sequoia Home Health and Hospice is seeking a compassionate Physical Therapist Assistant (PTA) to join our team in Santa Clara County. The PTA provides PT services under the direction of a licensed physical therapist and follows the physician's plan of care.

Responsibilities include administering direct therapy, guiding patients through exercises, documenting progress, and participating in home care team meetings. Per diem schedule and driving required; active CA PTA license and CPR are mandatory.

Formación

• Active California PTA license.
• Current CPR certification.
• 1+ year of clinical PTA experience.
• Must be a licensed driver with a reliable vehicle.

Responsabilidades

• Provide direct physical therapy per physical therapist's direction and plan of care.
• Treat patients to relieve pain and restore function; maintain maximum performance.
