Enrich the lives of older adults through community, choice, and continuing care in the Physical Therapist Assistant (PTA) role.
Under the supervision of a licensed Physical Therapist, the PTA will provide hands-on support to help patients improve mobility, regain independence, and enhance their overall quality of life. This role also supports hospice patients by promoting safety, comfort, and quality of movement in their home environment. Find your sense of belonging at Wesley!
Responsibilities include:
- Implement physical therapy treatment plans developed by the supervising Physical Therapist.
- Provide therapeutic exercises, gait training, and functional mobility interventions in patient homes.
- Monitor and document patient progress, reporting changes or concerns to the supervising PT.
- Collaborate with patients, families, and caregivers to promote active participation in care.
- Ensure proper use of equipment and assistive devices, educating patients and caregivers as needed.
- Support hospice care goals by promoting patient comfort and minimizing physical distress through gentle interventions.
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulations.
- May train patients in the use of prosthetic and/or orthotic devices.
- Communicate and collaborate effectively with the interdisciplinary care team to support holistic patient care.
- Ensure documentation accuracy and timeliness in the patient’s clinical record.
- Follow established policies and procedures.
- Participate in continuing education to stay informed of trends and maintain licensing requirements.
Requirements and qualifications:
- Associate degree from an accredited Physical Therapist Assistant program.
- Current and active Washington State PTA license.
- Current CPR & BLS certification.
- Experience in Home Health and/or Hospice preferred but not required.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Proficiency with electronic medical record (EMR) systems and mobile documentation tools.
- Kind, patient, and empathetic demeanor.
- Strong communication and teamwork skills.
- Self-motivated and capable of working independently in a home setting.
- Comfortable working with patients across the continuum of care, including end-of-life.
