Job summary

One Health Home Health and Hospice is seeking a Physical Therapist Assistant (PTA) to implement and monitor prescribed physical therapy care plans in Alameda County. The role focuses on helping patients regain mobility and independence, with travel to assigned home visits within a 20-mile radius.

The PTA will coordinate with physicians and the care team, document progress, and ensure high-quality outcomes in a home-health setting.

Qualifications

• Current and unrestricted California PT license required.
• Must have at least 1 year experience
• Current CPR certification
• Valid Driver's License
• Willingness to travel within a 20 miles radius to see patients.

The Physical Therapist evaluates and treats patients using medically prescribed physical therapy programs. The Physical Therapist may also supervise Physical Therapy Assistants per agency and regulatory standards.
