We are seeking a full-time Physical Therapist Assistant (PTA) or Physical Therapist (PT) to join our team in Portland, OR at Avamere Crestview of Portland. The team upholds a strong commitment to quality care, evidence-based practice, and outstanding outcomes.

As a Physical Therapy Assistant with Avamere Rehab, you will join a team with a culture and mission to enhance the life of every person we serve. Avamere Rehab provides in-house therapy in our Avamere Living communities throughout Oregon and Washington. Established in 1995, Avamere has grown to be the industry leader in the Pacific Northwest for Skilled Nursing and Post-Acute Care.   • Implement and execute patient treatment plans under supervision of a licensed PT
  • Communicate effectively with patients, families, physicians, and team members
  • Provide timely documentation per facility standards
  • Develop clinical skills through ongoing education and associations
  • Ensure documentation meets insurance, licensing, and regulatory requirements
  • Participate in department operations and development
  • Report changes in patient status promptly

Qualifications:

  • Graduate of an approved PTA program
  • Licensed as a PTA in Oregon
  • Ability to practice unencumbered and in good standing
  • Knowledge of Medicare and payer sources
  • Understanding of residents’ rights
  • Professional presentation and communication skills
  • Ability to multitask
  • Fluent in English (reading, writing, speaking)
  • CPR certification
