- Attractive Pay: Minimum Hourly Rate: $35.00, Maximum Hourly Rate: $42.00

Responsibilities
- Provides physical therapy services and interventions under the supervision of a physical therapist and in accordance with the physician-approved plan of care.
- Observes, records, and reports the patient's response to treatment and any change in the patient's condition to the supervising PT, physician, and other clinical staff.
- Provides resources for future needs and promotes wellness and prevention.
- Documents supervisory activities in the patient's medical record.
- Monitors and documents the patient's response to physical therapy intervention.
- Participates in patient care conferences, in-services, chart audits, quality improvement activities including OBQI and OBQM, and peer-review activities.
- Maintains confidentiality of patient information.
- Makes recommendations for follow-up care and utilizes other agency care providers for referrals to community resources.
- Performs other duties as assigned.
Qualifications
- Current license to practice as a physical therapy assistant in the assigned state.
- Graduate from an accredited PTA educational program as per CAPTE standards.
- One year of physical therapy experience for new PTAs entering home health; less than one year requires approval from regional leaders and mentoring or residency participation.
- Current CPR certification.
