Job summary

Charity Comfort Care Services, LLC is seeking a licensed Physical Therapist Assistant to provide skilled PTA treatments in patients' homes within the Dallas–Fort Worth area. The role offers flexible PRN and part-time opportunities, with assignments based on availability and local service area preferences.

You will work under a licensed Physical Therapist, implement treatment plans to improve mobility, and educate patients and caregivers on safe home exercise programs.

Qualifications

• Active Texas PTA license.
• Graduate of an accredited PTA program.
• Home health experience preferred.
• Experience with Axxess Home Health software preferred.
• Knowledge of Medicare home health documentation requirements.
• Excellent communication and time-management skills.
• Reliable transportation and willingness to travel to patient homes.
• Ability to work independently while under PT supervision.
• Current CPR/BLS certification preferred.

Responsibilities

• Provide skilled PTA treatments under supervision of licensed PT.
• Implement treatment plans to improve strength, balance, mobility, and independence.
• Assist patients with gait training, transfers, exercises, and fall prevention.
• Monitor patient responses and report changes to supervising PT.
• Educate patients and caregivers on safe mobility and home exercise programs.
• Assist patients recovering from surgery or hospitalization.
• Complete accurate visit documentation in Axxess Home Health.
• Communicate with physicians, nurses, therapists, and agency leadership.
• Follow Medicare home health requirements and agency policies.
• Maintain patient safety and provide compassionate care.

Key Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist (PT).
• Implement established treatment plans to improve strength, balance, mobility, and functional independence.
• Assist patients with gait training, transfer training, therapeutic exercises, and fall prevention.
• Monitor patients' responses to treatment and communicate changes to the supervising PT.
• Educate patients and caregivers on safe mobility, home exercise programs, and injury prevention.
• Assist patients recovering from surgery, hospitalization, illness, or mobility limitations.
• Complete accurate and timely visit documentation in Axxess Home Health.
• Communicate effectively with physicians, nurses, therapists, and agency leadership.
• Follow Medicare home health requirements, Texas regulations, and agency policies.
• Maintain patient safety and provide compassionate, professional care.

Qualifications
