Mohawk Meadows Rehab and Nursing Center seeks a dedicated Physical Therapy Assistant to support our interdisciplinary team in delivering high-quality rehabilitation for residents. You will work with licensed physical therapists to implement treatment plans, conduct therapies, and track resident progress.

You will contribute to education for residents and families, maintain accurate records, and participate in quality assurance to ensure safe, coordinated care in a dynamic clinical setting.

Responsabilités

• Assist with conducting physical therapy treatments, including exercises, evaluations, and assessments
• Assist with the development and implementation of treatment plans in collaboration with the physical therapist
• Provide education and training to residents, families, and caregivers on physical therapy techniques and exercises

About the Role

As a Physical Therapy Assistant at Mohawk Meadows Rehab and Nursing Center, you will work alongside our experienced physical therapists to provide individualized and effective physical therapy interventions to our residents. You will be responsible for assisting with the development and implementation of treatment plans, conducting physical therapy treatments, and maintaining accurate records of resident progress.
