Position Details

H2 Health Senior Services - 1:1 Patient Care with 5-15+ visits/week in ALF setting in Murray, KY. This role involves treating a primarily geriatric caseload within luxury senior living communities and residential visits within a 10-mile radius. You will work with general orthopedic conditions, vestibular rehab, neurologic movement disorders, and performance enhancement programs. The ability to work independently and in teams is essential, along with making appropriate decisions based on each patient's functional needs.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Licensed Physical Therapist Assistant in KY.
• Prior outpatient experience preferred, but new grads are welcome.
