M&Y CARE LLC in Cleveland, Ohio, is seeking a licensed Physical Therapist Assistant (PTA) for a full-time or part-time home care position. You will work under the supervision of a Registered Physical Therapist to provide patient services and help implement a rehab plan of care.

Responsibilities include documenting progress notes within 72 hours, assisting with assessments, and guiding patients through strengthening exercises and use of self-help devices.

• Perform routine strengthening and therapeutic exercises and assist the patient with self-help devices.

• As directed, perform routine strengthening and therapeutic exercises and assists the patient with the use of self-help devices.
