Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) to deliver skilled visits in patients' homes across Texas under a supervising PT. This field-based role emphasizes function, safety, and measurable progress, with a productivity-based compensation model.

You will work Monday–Friday with occasional weekends, travel throughout the assigned territory, and use Homecare Homebase (HCHB) for documentation. A Texas PTA license is required.

Qualifications

• Active Texas PTA license
• Strong clinical judgment in the field under PT supervision
• Comfort escalating changes to PT/RN/MD with clear rationale
• Strong communication and thorough documentation
• Reliable follow-through on treatment plans and goals
• Willingness to travel daily across assigned territory
• Ability to transport basic therapy equipment and perform core PTA scope activities (transfers, gait, balance, home safety)
• Experience with home health therapy tasks and patient/caregiver coaching

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
• Set and track objective progress within PTA scope; communicate updates to the supervising PT.
• Coordinate with the care team to elevate changes in condition and align on orders.
• Coach patients and caregivers with practical home programs that are feasible.
• Document promptly in Homecare Homebase (HCHB) with defensible and consistent records.

• Schedule: Full-time Monday-Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation (Productivity Points): $58.00 per point

The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes — in partnership with a supervising PT.

What you will do:

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.

• Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.

• Drive multidisciplinary coordination: elevate changes in condition, align on orders, and keep the care team moving.

• Coach patients and caregivers with practical home programs that actually get done.

• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

Must-haves

• Active Texas PTA license (in good standing)

• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)

• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on treatment plans, visit cadence, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health PTA scope

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device education

  • Strengthening, functional mobility, and home safety assessment/modification education

  • Patient/caregiver coaching with practical home programs

Nice-to-haves

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper clinical experience in high signal ortho/neuro populations

• Strong organization and time management
