Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist Assistant (PTA) to deliver in-home skilled visits across Texas under the supervision of a PT. This full-time role emphasizes gait training, functional mobility, home safety, and fall prevention, with travel across assigned territory.

You will be supported by a clinical and operational team, work within a productivity-based compensation model, and help advance access to high-quality home health care for patients in their homes.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field under PT supervision.
• Comfortable escalating changes in condition to PT/RN/MD with clear rationale.
• Excellent communication and documentation skills.
• Valid driver’s license and willingness to travel daily across assigned territory.
• Able to carry/transport basic therapy equipment.
• Competency in core PTA scope: gait training, transfers, balance, home safety, etc.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans with focus on functional mobility and progress: gait/balance, transfers, strengthening, home safety, fall prevention.
• Set/track objective progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB — defensible, efficient, and consistent.

Physical Therapist Assistant (PTA) - Home Health

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)
• Location: Field Based
• Compensation (Productivity Points): $58 per point/30 points per week

Requirements

Must-haves

• Active Texas PTA license (in good standing)
• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)
• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly
• Strong communication and documentation
• Reliable follow-through on treatment plans, visit cadence, and patient goals
• Current driver’s license and comfort with daily travel across assigned territory
• Able to carry/transport basic therapy equipment as needed
• Competency in core home health PTA scope
