Adaptive Home Health is seeking a Licensed Physical Therapist Assistant to provide skilled PTA visits in patients' homes within the Houston area, under PT supervision, following a defined plan of care.

This full‑time role offers a productivity-based pay model (58.00 per point) with 30 points/week; strong emphasis on mobility gains, documentation, and coordination with the care team to ensure timely, safe patient progress.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field under PT supervision.
• Comfort escalating changes in condition to PT/RN/MD with clear rationale.
• Strong communication and thorough documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope (gait, balance, transfers, safety).

Responsibilities

Licensed Physical Therapist Assistant (PTA) - Home Health

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based (Houston pod)

• Compensation (Productivity Points): $58.00 per point

What you will do:

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.

• Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.

• Drive multidisciplinary coordination: escalated changes in condition, align on orders, and keep the care team moving.

• Coach patients and caregivers with practical home programs that actually get done.

Requirements

Must-haves

• Active Texas PTA license (in good standing)

• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)

• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly

• Strong communication and documentation
