Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) to deliver skilled visits in patients' homes under a supervising PT. You will help restore function, enhance safety at home, and drive measurable mobility gains.

This full-time, field-based role operates Monday–Friday with occasional weekends, and uses a productivity-based pay model: $58.00 per point or $75k–$100k per year. Travel across assigned territory is required, with documentation in Homecare Homebase.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field under PT supervision.
• Comfort escalating changes in condition to PT/RN/MD with clear rationale.
• Strong communication and documentation skills.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and ability to travel daily across territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope: mobility, gait, balance, safety.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait, transfers, strengthening and home safety.
• Set/track objective progress and communicate updates to the supervising PT.
• Drive multidisciplinary coordination and keep the care team aligned.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).

The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes - in partnership with a supervising PT.

We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier - protecting your time.

What you will do:

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.

• Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.

• Drive multidisciplinary coordination: elevate changes in condition, align on orders, and keep the care team moving.

• Coach patients and caregivers with practical home programs that actually get done.

• Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent.

Requirements

Must-haves

• Active Texas PTA license (in good standing)

• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)

• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on treatment plans, visit cadence, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health PTA scope

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device education

  • Strengthening, functional mobility, and home safety assessment/modification education

  • Patient/caregiver coaching with practical home programs

Nice-to-haves

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper clinical experience in high-signal ortho/neuro populations
