Job summary

Putnam Nursing & Rehabilitation Center in Putnam County, NY is seeking a Physical Therapist Assistant (PTA) for a full-time role in our skilled nursing and rehabilitation facility. You will work under the supervision of a Physical Therapist to implement and monitor therapy programs, assist with resident care, and document progress.

Requirements include graduating from an accredited PTA program and NY State PTA license in good standing; SNF experience is preferred.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Licensed in NY as a PTA in good standing.
• Previous skilled nursing facility (SNF) experience preferred.
• Work well under pressure & willing to work harmoniously with all staff.
• Strong communication with residents, families and staff.
• Covid-19 vaccinations encouraged but NOT mandated; vaccination guidance followed by CDC.

Responsibilities

• Organize and conduct physical therapy programs.
• Assist in planning, organizing, developing and implementing Physical Therapy services under the supervision of the Physical Therapist.
• Perform treatment programs for assigned residents.
• Enter billing information accurately and timely.
• Report resident progress in UR meetings and family conferences as required.
• Consult with other rehabilitation team members to coordinate therapy with other activities.
• Provide residents with equipment as appropriate.
• Chart notes documenting therapy provided and resident progress.
• Cooperate with all departments and staff to promote optimal resident care.

• Physical Therapist Assistant, PTA is responsible to organize, and conduct physical therapy programs planned and overseen by the Physical Therapist to facilitate rehabilitation of those residents who have experienced a decrease in functions.
• Physical Therapist Assistant, PTA will assist in planning, organizing, developing and implementing Physical Therapy services under the supervision of the Physical Therapist, to ensure that the highest degree of quality resident care can be maintained at all times.
• Physical Therapist Assistant, PTA performs treatment programs for assigned residents
• Physical Therapist Assistant, PTA enters billing information in an accurate and timely manner
• Report resident progress in meetings, i.e. UR Meeting, Patient/Family Conference as deemed required or appropriate
• Consult with other members of the rehabilitation team to coordinate physical therapy with other therapeutic activities
• Provide residents with equipment as appropriate
• Chart notes in an informative and descriptive manner that reflects the therapy provided to the residents, as well as the resident’s response to the therapy
• Perform routine documentation as required and in accordance with our established charting and documentation policies and procedures. Sign and date all entries made in residents’ medical record.
• Cooperate with all departments and staff facilitating harmonious working conditions to promote optimal resident care

Requirements

• Graduate of an accredited school of Physical Therapy Assistant, PTA
• Licensed in NY State as a Physical Therapist Assistant, PTA; license in good standing
• Previous skilled nursing facility (SNF) experience highly preferred
• Work well under pressure & willing to work harmoniously with all staff
• Sincere desire to work with the elderly, chronically ill, and handicapped
• Excellent customer service skills
• Must be able to communicate well verbally with residents, families and staff
• Treat residents and fellow staff with respect, dignity, and compassion
• Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC
