Job summary

Cedarbrook Health & Rehab is seeking a compassionate Physical Therapist Assistant (PTA) for a per diem role in a skilled nursing facility. You will support rehabilitation efforts, assist with mobility and therapeutic exercises, and help residents achieve independence.

Qualifications

• Completion of an accredited Physical Therapist Assistant program.
• Strong communication and interpersonal skills, especially with elderly or rehabilitating residents.
• Ability to work collaboratively with Physical Therapists, nursing staff, and other care team members.
• Compassionate, patient-centered approach to care.
• Basic understanding of rehabilitation principles and SNF regulatory guidelines (preferred).
• Ability to lift, support, and assist residents safely during mobility and therapeutic activities.

Responsibilities

• Deliver high-quality patient care under the supervision of a licensed Physical Therapist.
• Assist residents with mobility training, gait training, transfers, and strengthening exercises.
• Implement individualized therapy treatment plans according to resident needs.
• Monitor resident progress and document outcomes accurately and timely.
• Provide education and support to residents and their families regarding therapeutic activities and home exercise programs.
• Maintain a safe and clean therapy environment, ensuring equipment is used appropriately.
• Collaborate with nursing staff, therapy teams, and interdisciplinary teams to support overall care plans.
• Encourage resident participation and promote independence in daily functional activities.

In a skilled nursing facility (SNF) setting, PTAs play a vital role in supporting residents through their rehabilitation journeys. Key Responsibilities
