Job Responsibilities

• Communicate daily with office staff to accept new patients
• Complete paperwork and submit payroll
• Maintain open communication with patients and home health agencies
• Coordinate with Physical Therapists to manage patient care
• Complete daily visit notes and paperwork
• Attend training and team meetings as needed

Job Requirements

• Current Physical Therapist Licensure for Michigan
• Degree in Physical Therapy from an accredited program
• Ability to travel to patients' homes or facilities
• Strong communication skills
• Home healthcare experience preferred
