Job summary

The Center at Blue Hills in Stoughton, MA is seeking a full-time and PRN Physical Therapist Assistant to join our in-house therapy team caring for residents with Alzheimer's and Dementia. You will provide hands-on therapy under an established plan of care and collaborate with a multidisciplinary team.

Qualifications

• Graduate PTA program completed and in good standing.
• Current state PTA license in good standing.
• CPR/BLS certification required.
• Experience in skilled nursing or subacute rehab preferred.
• Strong interpersonal and communication abilities.

Responsibilities

• Administer treatment to residents to relieve pain and improve function.
• Deliver therapy in accordance with the established plan of care.
• Instruct patients, families, and caregivers on therapy techniques.
• Monitor and document patient responses to treatment.
• Complete required forms and maintain documentation per policy.
• Collaborate with patients to assess conditions and goals.
• Develop and adjust treatment plans using various techniques.
• Create personalized mobility and functional fitness programs.
• Provide education on injury prevention and home exercises.
• Coordinate with other healthcare professionals as needed.

The Center at Blue Hills located in Stought, MA has an exciting opportunity available for a Full-time and PRN Physical Therapist Assistant (PTA) to join their new in-house therapy team!

The Center at Blue Hills is proud to provide care for and specialize in Alzheimer's Care and Dementia Care.

63 Beds, Skilled Nursing Facility with primarily Psych and Dementia caseload.

Responsibilities

• administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Assure all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Monitor patient response to treatment intervention
• Complete required forms and documentation in accordance with company policy and state/federal regulations
• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized fitness-oriented health care programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain and improve mobility.
• Advising patients on exercise techniques.
• Advising patients and their families about in-home treatment options.
• Providing educational information about injury prevention, ergonomics and ways to promote physical health.
• Consulting and collaborating with other healthcare professionals.
• Documenting patient care history.
• Complying with rules, regulations, and procedures.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current state PTA license in good standing.

• Previous experience in a skilled nursing or subacute rehab setting preferred.
