American Senior Communities is seeking a PRN Physical Therapist Assistant (PTA) to provide skilled care under the supervision of a registered physical therapist. The role supports patients across ASC Therapies & Wellness settings including home health, skilled nursing, and outpatient services in Kokomo, Peru, and Wabash.

You’ll develop and adjust treatment plans, assist with discharge planning, and contribute to case management while upholding ASC standards.

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.
