American Senior Communities seeks a PRN Physical Therapist Assistant (PTA) to provide skilled care under the supervision of a registered physical therapist across ASC Therapies & Wellness settings, including home health, SNF, and outpatient settings in South Bend, Mishawaka, and Elkhart.

The PTA will deliver direct therapy per the OT plan of care, monitor patient progress, and assist with discharge planning and case management, while upholding state regulations and delivering compassionate care

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.
