CONSUMER DIRECTED SERVICES INC. is seeking a Physical Therapy Assistant to provide skilled education, treatment, and interventions in a home health setting, following the Physician's plan of care.

You will deliver direct physical therapy services under the supervision of a licensed physical therapist and physician, communicate with patients and caregivers about the care plan, and support safe, functional outcomes in patients' homes.

Qualifications

• Graduation from an accredited Physical Therapist Assistant program.
• Current, unrestricted state licensure.
• Valid CPR certification.
• Professional and ethical conduct, with adherence to confidentiality and documentation standards.

Responsibilities

• Delivering direct physical therapy services in accordance with directions from the physical therapist and physician.
• Communicating regularly with patients and caregivers about the care plan.
• Providing treatments, procedures, and education to ensure patient safety and optimal function.
• Reporting patient observations and changes in condition to supervisors and physicians.
• Teaching patients, families, and caregivers on the safe use of prosthetic, orthotic, and assistive devices.
• Collaborating with interdisciplinary team members to discuss and plan patient care.
• Planning for discharge and providing relevant instructions to patients and caregivers.
• Maintaining documentation, clinical notes, and ensuring confidentiality per professional standards.
• Participating in in-service programs and other job-related duties as assigned.

Education
