Sangre de Cristo Community Care in Pueblo, Colorado, is seeking a compassionate Full-Time Physical Therapist Assistant to join our growing home health team. You will deliver skilled therapy under a licensed PT, helping patients improve mobility and quality of life in their homes.

Responsibilities include providing therapy per treatment plans, documenting visits, reviewing diagnoses, and communicating progress to the supervising PT.

Qualifications

• Must be at least 21 years old.
• Graduate of an accredited Physical Therapy Assistant program culminating in an associate degree.
• Must be licensed or eligible for licensure in Colorado.
• Must have a valid non-restricted driver’s license.
• Flexibility and availability to work a weekend schedule if needed.
• Two years of appropriate experience as a Physical Therapist Assistant.
• Willing to visit patients throughout Otero and surrounding counties.

Sangre de Cristo Community Care is seeking a compassionate and skilled Full-Time Physical Therapist Assistant (PTA) to join our expanding Pueblo Home Health team. As our Home Health services continue to grow in Pueblo, this is an exciting opportunity to support patients in their homes while helping build and strengthen care in the community.

• Pay range: $30–$34 per hour, based on experience

About the Role

As a Physical Therapist Assistant, you will provide skilled therapy services under the supervision of a licensed Physical Therapist. You will play an important role in helping patients improve mobility, function, and overall quality of life through individualized, patient-centered care delivered in the home setting.

Key Responsibilities

• Provide physical therapy services in accordance with established treatment plans and under the supervision of a licensed Physical Therapist
• Complete visit documentation accurately and in a timely manner
• Review patient diagnoses and conditions, observing all indications, contraindications, and required precautions
• Communicate patient progress, concerns, and changes in status promptly to the supervising Physical Therapist
