Job summary

Live Well Homecare seeks a dedicated Physical Therapy Assistant to deliver in-home therapy under a supervising Physical Therapist’s plan of care. You’ll work with patients, families and caregivers to promote safe mobility and participation in daily activities.

Your duties include instructing patients in exercises, recording responses, reporting changes, and coordinating with the health team to ensure quality care within legal boundaries. Weekend availability is preferred.

Job description

The role involves providing physical therapy assistance to patients in their homes, following a treatment plan established by a supervising Physical Therapist.

The assistant collaborates with healthcare team members and educates patients, families, and caregivers within legal and scope boundaries.

Responsibilities

• instructing patients in exercises, muscle re-education, gait, ADLs, transfers, and safety, as well as observing, recording, and reporting patient responses and condition changes.

Qualifications

• a valid physical therapy assistant license, a valid driver’s license, and availability for weekend work.

Preferred Qualifications

• current CPR certification and the ability to perform physical tasks like lifting and assisting mobility.
