Job Summary

As a Physical Therapist Assistant I, you will work under the supervision of a licensed Physical Therapist to execute treatment plans for homebound patients. This role requires independence, strong clinical skills, and the ability to adapt to diverse home environments while adhering to Medicare and ICAHO guidelines.

Key Responsibilities

• Implement treatment plans as directed by the supervising Physical Therapist, ensuring adherence to frequency and duration specifications.
• Document patient progress, strength, ROM, balance, and functional mobility using objective measures.
• Communicate effectively with patients, caregivers, physicians, and the rehab team to coordinate care.
• Conduct discharge planning and make referrals for ongoing services as needed.
• Provide patient/caregiver education and address safety needs.
• Maintain accurate and timely documentation in compliance with agency, state, and federal regulations.
• Participate in continuing education, in‑services, and departmental initiatives.

Qualifications

• Education: A.A. Degree from an approved Physical Therapist Assistant program.
• Licensure/Certification:
  • Current Maryland Physical Therapist Assistant License.
