Responsibilities

• Provides physical therapy services under the supervision of a licensed physical therapist, following the physician's plan of care.
• Observes, records, and reports patient's responses and any changes in condition to the supervising PT, physician, and clinical staff.
• Promotes wellness and prevention, providing resources for future needs.
• Documents supervisory activities in the patient's medical record.
• Monitors and documents patient responses to interventions.
• Participates in care conferences, in-services, audits, quality improvement, and peer-review activities.
• Maintains confidentiality of patient information.
• Makes recommendations for follow-up care and coordinates with other care providers and community resources.
• Performs other duties as assigned.

Qualifications

• Current license as a physical therapy assistant in the assigned state.
• Graduate from an accredited PTA program as per CAPTE standards.
• At least one year of relevant home health experience; less than one year requires approval from regional leaders and mentoring or residency participation.
• Current CPR certification.
