A leading long-term care facility in Worcester is seeking a Physical Therapy Assistant to assist in the planning and implementation of therapy programs. The role involves direct patient care, documentation, and collaboration with the rehab team to ensure quality resident care.
Physical Therapy Assistant Responsibilities :
Assists in the planning, development, organization and implementation of physical therapy as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
Participates in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
Performs administrative requirements such as completing necessary forms, reports and monitors efficiencies on an ongoing basis in collaboration with Rehab Director.
Prepares resident for treatment and administers physical therapy in accordance with established policies and procedures which includes one on one, supervised and group treatment.
Assures that all notes are informative and descriptive of the care provided and of the resident’s response to the care. Documents resident’s progress towards meeting established goals.
Consistently demonstrates an awareness and concern for the safety, property, and confidentiality of patients, visitors, and co-workers.
Provides privacy and maintains the confidentiality of all resident / patient care information.
Requirments :

Must possess a current state license
