Azza HealthCare Agency is seeking a dedicated Physical Therapist Assistant for in-home care to implement treatment plans, guide exercises, and educate patients for improved well-being.

Responsibilities include following licensed PT plans, assisting with activity and gait training, monitoring patient responses, educating families, and coordinating with the care team. Weekend availability is required in a supportive, growth-focused environment.

Qualifications

• Current license, registration, or certification to practice physical therapy assistance in the state.
• CPR certification, valid driver’s license, and reliable transportation.
• Ability to work in diverse home environments and perform physical tasks such as lifting and transferring.
• Availability for weekend rotations.
