Job summary

Presbyterian Homes & Services (PHS) seeks a Full-Time Therapist Assistant at Folkestone in Wayzata, MN. You will provide treatment under plan of care, educate residents and families, and document care with our in-house therapy team.

This is a chance to serve across multiple settings, including SNF, assisted living, and home health, with strong team support.

Qualifications

• Graduate from an accredited Physical Therapist Assistant program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

• Provide treatment per the established plan of care and in accordance with physician orders.
• Assist in identifying goals, prioritizing treatment, and selecting appropriate interventions.
• Continuously assess changes in functional deficits and adjust treatment as appropriate.
• Provide education to residents/clients, families, caregivers, and staff.
• Collaborate with therapist on observations, progress towards goals, and recommended updates to plan of care and discharge.
• Provide accurate and timely documentation & billing in electronic medical record.
• Demonstrate understanding and knowledge of payer sources and regulatory requirements.
• Coordinate with therapist on supervisory visits per regulations.
• Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.

Folkestone is seeking a Full-Time Therapist Assistant.

SCHEDULE: Monday-Friday, no weekends

Pay Range

The estimated starting pay range for this role is $30.24 - $34.57 per hour. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.

Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
