Job description

Overview

Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress through observations and follows up with supervising therapist. Works under PT supervision as required under NY State Education law.

Responsibilities include:

1. Following established physical therapy plan of care, including interventions and patient/caregiver education. Modifies procedures in accordance with PT plan and PTA scope of practice.
2. Documenting all care provided and maintaining records per VNS Health policy.
3. Monitoring patient progress and communicating with the Supervising Therapist as per policy.
4. Managing assigned caseload and providing services as directed by the PT plan of care.
5. Participating in meetings, training, and caseload reviews.
6. Traveling to patients’ homes or facilities, adapting to various environments.
7. Assisting with therapeutic interventions, activities of daily living, and use of medical equipment.
8. Adhering to agency policies, practices, and professional standards.
9. Transporting and utilizing technology devices and supplies.
10. Participating in special projects and other duties as assigned.

Qualifications

Licenses and Certifications: New York State Certification as a Physical Therapist Assistant required. Valid driver’s license may be required based on operational needs.

Education: Associate's Degree from an approved NY State program or equivalent, required.

Work Experience: Minimum of two years in clinical patient care, required. Bilingual skills and basic PC skills preferred.

Pay Range

USD $30.71 - USD $38.41 /Hr.
