Phoenix Home Care & Hospice in Missouri is seeking a licensed Physical Therapist Assistant to provide PT services in patients' homes, under the supervision of a licensed Physical Therapist, with weekday visits in Eldon, Versailles, Stover, MO and surrounding areas. Qualifications

• MO PTA certification is required.
• Home Health experience is preferred.
• Ability to pass drug test, background check, and physical.

Responsibilities

• Provides PT assigned services to patients in the home setting under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote continuity and quality of care.
• Exhibits sound judgment and technical skills to perform treatments per agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates compliance with agency standards, state/federal regulations, and payer requirements.
• Performs safety, infection control, and other job-related duties as assigned.

• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.

Requirements

• Physical Therapist Assistant Certification for the state of MO
• Home Health experience preferred, not required
