Therapy Management Corporation (TMC) is seeking a Licensed Physical Therapist Assistant to deliver patient-centered physical therapy for adults and geriatric patients. You will work under the supervision of a PT, implementing individualized treatment plans and tracking progress in collaboration with a multidisciplinary team.

Qualifications

• Licensed PTA or eligible in the state of practice (IL).
• Graduate of an accredited PTA program.
• Interest or experience in geriatric rehabilitation or senior living therapy.

Responsibilities

• Provide personalized physical therapy under the direction of a PT to improve mobility and functional independence.
• Implement patient-centered treatment plans to support recovery and safety.
• Deliver hands-on therapy to enhance movement and endurance for adults and seniors.
• Collaborate with interdisciplinary teams, caregivers and nursing staff for outcomes.
• Maintain accurate clinical documentation and adjust plans as needed.

• Implement patient-centered treatment plans designed to support recovery, safety, pain management, and long-term rehabilitation goals
• Deliver hands-on therapy interventions that enhance movement, endurance and overall quality of life for adult and geriatric patients
• Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care
• Maintain accurate clinical documentation, monitor patient progress and adjust treatment plans based on individual rehabilitation needs

Qualifications
