Performance Optimal Health is seeking an Outpatient Physical Therapist Assistant to join our team in Connecticut. You will work with patients along their Optimal Health journey, delivering individualized treatment plans under supervision and collaborating with a multidisciplinary team.

The role emphasizes clinical excellence, patient education, and documentation, with opportunities to grow as the network expands within the Naples area and beyond.

Qualifications

• 0-4 years’ experience and must possess a valid license (Open to recent graduates).
• Associates degree from PTA program is required.
• Licensure as a Physical Therapist Assistant is required to practice in the U.S.

Responsibilities

• Be an integral part of the Client Experience and work with patients throughout their Optimal Health journey.
• Ensure clinical excellence through assessments, mentorship, and continuing education.
• Provide top-notch, empathic clinical care and implement patients’ individualized treatment plans.
• Instruct patients, families, and caregivers in PT techniques under the supervision of the Physical Therapist.
• Collaborate with the Optimal Health Team to implement programs matching patient needs.
• Maintain daily patient chart documentation of interventions and services provided.

Join Performance Optimal Health as an Outpatient Physical Therapist Assistant. With just over 20 years of experience throughout Connecticut and New York, we have successfully launched our first site in Naples, Florida. We are excited to continue growing and building upon our Physical Therapy Team. We are seeking an individual with a passion for health and wellness, and a willingness to work within a team of individuals focused on impacting the lives of our clients in a positive way. • 0-4 years’ experience and must possess a valid license (Open to recent graduates).
