Position Details:

• Sign-on bonus: $2,500!

Purpose of this position: To treat and document care for residents with impaired ability to perform independently due to disease, under supervision of a physical therapist; participate in interdisciplinary team meetings.

Responsibilities include:

• Follow physicians’ orders and physical therapist instructions.
• Provide treatment to help residents maintain or attain optimal function.
• Implement preventative measures and adaptive equipment.
• Maintain accurate reports as required by regulations and payers.
• Observe infection control and safety procedures.
• Work cooperatively with residents, staff, physicians, and other providers.
• Follow facility policies, safety, privacy, and confidentiality guidelines.
• Present a professional appearance and dependable attendance.
• Perform duties effectively and timely, including other tasks as assigned.

Qualifications:
