Care Resources PACE in Grand Rapids, MI is seeking a full-time Physical Therapy Assistant (PTA) to join a caring clinical team. You will deliver PT services under a PT's supervision, Monday–Friday during daytime hours, with travel to client homes within Kent County in addition to clinic/day center work.
The ideal candidate has a PTA license and at least 1 year of PTA experience, with a focus on serving frail or elderly participants.

Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) which is located at 4150 Kalamazoo Avenue SE in Grand Rapids.Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, social and recreational activities at the Day Center. To be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home and meet the nursing facility's level of care.
