Alternate Solutions Health Network is seeking a Physical Therapist Assistant I (PTA I) to provide comprehensive skilled services under the direction of a Physical Therapist in patients' homes, documenting care and ensuring safety and regulatory compliance.

The PTA I educates patients and families, communicates with physicians and caregivers, and participates in case conferences to promote coordinated care while striving to restore function and relieve pain.

Qualifications

  • Must have at least 1 year of PTA experience, preferably in home health or acute settings.
  • Valid Physical Therapist Assistant license in the state of employment.
  • Excellent communication skills with patients, families, physicians and co-workers.
  • Detail-oriented, reliable, and able to manage documentation and compliance.

Responsibilities

  • Provide skilled PTA services in patients' homes as directed by the PT plan of care.
  • Observe, document, and report patient progress and safety concerns.
  • Perform PT procedures and modalities prescribed by the therapist.
  • Educate patients and families on therapy techniques and safety.
  • Collaborate with physicians and care team; participate in case conferences.
  • Complete clinical documentation within agency guidelines and Medicare rules.

The Physical Therapist Assistant educates patients and their family members and ensures safety of the patient. The PTA I is accountable for completing accurate documentation and remains compliant with all legal rule and regulations.
