Job summary

Trilogy Home Healthcare, a Humana company, is seeking a dedicated Certified Physical Therapy Assistant to provide home health visits in West Volusia County, FL. This PRN/Per Diem role focuses on one-on-one care, following physician orders, and improving patients' independence and quality of life.

You will work under a Registered PT, thrive in a supportive, team-driven environment, and contribute to a trusted, award-winning home care team with flexible scheduling and opportunities for growth.

Qualifications

• Current PTA license/certification in Florida.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ year of PTA and home health experience preferred.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Follow physician orders and treatment plans under supervision of a Registered PT.
• Deliver therapy to help patients achieve independence and improve quality of life.
• Collaborate with a team to ensure patient satisfaction and outcomes.
