Physical Therapist Assistant (PTA) – Home Health Care & Hospice
Location: Field-based, serving patient homes in designated areas
Job Type: Full-Time or Part-Time

Implement treatment plans designed by the physical therapist to assist patients in regaining mobility and function.
Provide hands-on therapy and instruct patients on therapeutic exercises and techniques.
Monitor patient progress and report changes to the supervising physical therapist.
Document patient care accurately and in compliance with regulatory standards.
Educate patients and caregivers on home exercises and safe mobility practices.
Active Physical Therapist Assistant (PTA) license in the State of Michigan.
Experience in home health care is preferred but not required.
Strong communication and interpersonal skills.
Ability to work independently while collaborating with the care team.
Commitment to delivering patient-centered care with attention to detail.
