Key Responsibilities

• Screen and evaluate patients with cognitive and ADL physical functioning disorders, developing appropriate care plans following standards.
• Use standardized assessments and evidence-based practices to support interventions.
• Provide rehabilitative, skilled, and medically necessary treatments for patients with physical functioning disorders.
• Administer procedures such as ADL retraining, therapeutic exercises, modalities, and use of assistive devices.
• Assess patient progress and modify treatments accordingly, documenting in clinical records.
• Evaluate treatment effectiveness and modify services based on outcomes and referrals.
• Offer consultation and counseling to patients, families, and caregivers regarding physical disorders.
• Communicate effectively with team members about patient progress and treatment plans.
• Prepare comprehensive discharge summaries per regulatory standards.
