Are you interested in a Float Pool opportunity? Join our dynamic WellStar Enterprise Support Team (WEST) and become a vital part of ensuring seamless staffing across WellStar facilities! As a member of the WEST Team, you'll have the chance to float to various units based on the greatest need, with assignments determined by WFE leadership, considering your clinical expertise and skill set.
The Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs to improve mobility, relieve pain, increase strength, and prevent deformities due to disease or injury. Services are provided across the continuum of care in a cost-effective, quality-focused, and customer-oriented manner, complying with all regulatory standards.
Core Responsibilities and Essential Functions:
- Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans.
- Develop a care plan with input from the patient and/or family.
- Create a realistic discharge plan incorporating patient and/or family goals.
Treatment Planning and Care Provision:
- Implement physical therapy treatment plans.
- Select appropriate activities to advance patient goals.
- Revise treatment goals based on patient and family feedback.
- Provide education and training to patients and families.
- Offer discharge instructions, follow-up, and referrals to community resources.
- Act as a referral source for staff, families, and physicians regarding rehabilitation services and equipment.
Documentation:
- Document evaluations and care plans per departmental guidelines.
- Record each treatment session as per protocols.
- Complete and submit family education, discharge, and other documentation in line with facility and program standards.

Minimum Education:
- Graduate from an accredited physical therapy program.
- Doctorate in Physical Therapy required.
Licenses and Certifications:
- Physical Therapist license required.
- Basic Life Support (BLS) certification from AHA required.
Experience:
- Minimum of 3 years of acute or inpatient rehab experience as a Physical Therapist required.
