LiveWell Homecare Agency, LLC seeks a licensed Physical Therapist to provide skilled inpatient services in an acute care setting, collaborating with a renowned medical team to support a diverse patient population.

You will establish and execute individualized treatment plans, educate patients and families, delegate tasks, complete documentation, and perform assessments and consultations. Licensure as a PT is essential; bachelor's degree required.

Qualifications

• PT licensure is required.
• Bachelor's degree in Physical Therapy (PT) is required (Master's preferred).
• No specific prior experience is required, but licensure is essential.

Responsibilities

• Participate in inpatient acute care with a multidisciplinary team.
• Educate patients, families, and other healthcare providers; refer to specialists as needed.
• Complete documentation in line with department policies.
• Perform assessments, reassessments, and consultations for PT services.

• Performs assessments, reassessments, and consultations for physical therapy services.

Qualifications & Skills:

• Bachelor's Degree in Physical Therapy (PT); Master's degree preferred.
• License as a Physical Therapist.
• No prior experience required, but licensure is essential.
