Job summary

Greenfield Rehabilitation Agency, Inc. is seeking a Physical Therapist (PT) to join our therapy team in Wisconsin.

We offer full-time, part-time, and PRN opportunities with a sign-on bonus for eligible full-time clinicians, serving seniors in skilled facilities and in-home care environments. Responsibilities include evaluations, designing individualized plans, fall prevention, mobility training, and collaboration with interdisciplinary teams.

Qualifications

• Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
• License as required by the State of Wisconsin to practice as a Physical Therapist.

Responsibilities

• Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance.
• Identify barriers to functional independence and design individualized physical therapy programs.
• Address challenges related to mobility, posture, gait, and injury or illness recovery.
• Provide interventions to help residents regain strength, prevent falls, and maintain independence.
• Support residents in daily functional tasks, recommending adaptive equipment as needed.
• Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function.
• Assess living spaces to recommend modifications for safety and accessibility.
• Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention.
• Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care.
• Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies.
